Apply:
-Submit an online application through Health-e-Arizona Plus.
-Download the Application for Benefits (FAA-0001A) and turn it in by mail or fax.
-Call 855-432-7587 to request a paper application be mailed to you. After receiving and completing the application, turn it in by mail or fax.
-Go to find an assistor, enter your zip code to locate community assistors near you for help with the application process. Contact the community assistor's office by telephone. In-person services may be unavailable or limited in manners that support social distancing.

CSFP: Commodity Supplemental Food Program
 
Monthly box may include a varietyof the following:  juice, farina, oats, ready-to-eat cereal, rice, pasta, peanut butter, dry beans, canned meat, poultry, or fish, and canned fruits and vegetables. 
 
Eligibility: Elderly that are at or below 130% of the Federal Poverty Income Guidelines.

-Pre-made meals available for purchase and/or in some cases covered by Medicare.
-Usually shipped in batches of 10, 14, and 21 days.
-Can store up to 14 days in refrigerator.
-Costs range around <$8.00/meals. Discounts offered for bulk orders and veterans.
 
-Most packages are priced at 2 to 4 servings.
-There are some single-serve options.
-Pre-made meals available for purchase and/or in some cases covered by Medicare.
 

Lunch at Aster Aging Senior Centers

Hot, nutritious lunches are served weekdays at 11:30 am at our Senior Centers. Reservations are required.

For next day reservations (lunch is served at 11:30 am), call 480-962-5612 (Mesa Downtown Senior Center) or 480-218-2221 (Red Mountain Senior Center).

The Suggested Contribution for participants age 60+ is $3.50.
