      What are the MetaPhy CCM billing codes?

      • Chronic Care Management (CCM)
        • 99490 – 20+ minutes of CCM time
        • 99490 + 99439 – 40+ minutes of CCM time
        • 99490 + 99439(x2) – 60+ minutes of CCM time (Note: Only submit one 99439 record, but set the number of units to 2)
      • Remote Patient Monitoring (RPM)
        • 99453 – RPM device (scale) setup
        • 99454 – RPM daily monitoring (device data transmission)
        • 99457 – RPM monthly monitoring (20+ minutes of RPM time)
        • 99457 + 99458 – RPM monthly monitoring (40+ total minutes of RPM time)
